    I certify that the person, partnership, or joint venture who owns the mentioned property has given permission to request water and sewer availability from Harpeth Valley Utilities District.

    Notes
    1. Provide a site plan or subdivision plat with request form if available.
    2. If a property does not have road frontage, an Agreement regarding the Maintenance of Private Service Lines by the Property Owner will need to be executed.
    3. If site is to be subdivided or it is not in an existing subdivision with existing utilities, then an availability review fee is required.

    Water and Sewer Availability Fees (non-refundable) Amounts are as follows:
    A. Single Family Residential and Small Residential Developments (1-4 Lots) $150.00
    B. Commercial, Multi-Family and Residential Development (5 or more Lots) $475.00
